Do you accept my dental benefits plan?

DeForest Dental is an “in-network” provider with Delta Dental, Cigna and Humana.  We still accept and file claims to almost all other dental insurance companies at the out-of-network level to provide you with the best possible coverage available.

Delta Dental – We are “in-network” for both the PPO and Premier levels of plans.  We offer the highest amount of coverage for those with Delta Dental insurance.

“In-Network” versus “Out-of-Network” Coverage:

  • “In-Network” – We have signed a contractual agreement with the insurance company to provide their insured patients “discounted” fees. The advantage of working with an “in-network” provider is the patient receives the lower negotiated rate and the insurance generally covers a larger portion of the bill than with an “out-of-network” provider.
  • “Out-of-Network” – The provider does not have a contractual agreement with your insurance company.  Some plans may not reimburse “out-of-network” providers at all, where other plans offer coverage for “out-of-network” providers; however, your patient responsibility may be higher than it would be if you were seeing an “in-network” provider.

We are happy to do a complimentary benefits check – if you have questions, give us a call!  We will need some basic information from the policy holder (name, insurance company, birthday, group number, ID number and/or SSN).  We are happy to help you any way we can!

We offer a 6-month and 12-month interest-free program available through Care Credit.  Please call us for further information.


We offer a 5% discount for same-day payment with cash or check.  This discount cannot be in conjunction with an insurance policy or a finance plan.


We offer the highest standard of care at a reasonable cost; therefore, payment and co-payment are due at the time services are rendered unless other arrangements have been made in advance.  The patient is fully responsible for all fees charged by our office, regardless of insurance coverage.  We do our best to estimate insurance coverage based on the information your insurance company provides; however, any remaining balance after your insurance has paid is your responsibility.
